ACL and tendon grafts

We are experiencing problems with our appeals for a tendon graft (20924) when we bill with CPT code 29888 ACL reconstruction. We have tried different avenues but have not been able to have the denial overturned. What strategy should we take?

This is a good question. The ACL reconstruction is valued to include the tendon work as the tendon graft is an integral part of the procedure.  Since an ACL is an arthroscopically assisted procedure, the surgeon will have a “separate incision’  for the harvest of the tendon thus the work associated with the harvest of the tendon graft from the same extremity is included. If the surgeon uses tendon allograft, the surgeon does not have to append modifier 52 even though there  is no work associated with harvesting a tendon.
